Neurotoxin Injections (BOTOX® Cosmetic and Dysport™)





If your brow and forehead have permanent lines and you look like you are frowning even when you are not, you may be giving the wrong impression to others around you. There are many options for improving forehead wrinkles, including a surgical brow lift. This invasive procedure, however, is not always necessary Dr. Doherty offers his Boston patients an alternative: Dysport™ and BOTOX® Cosmetic injections.

Both Dysport™ and BOTOX® are neurotoxins. The injection is a simple non-surgical procedure that smoothes the deep, persistent facial wrinkles caused by hyperactive muscles beneath the skin. Dysport™ and BOTOX® Cosmetic have been aggresively tested and is approved by the FDA. Neurotoxin injections have no scarring and result in minimal downtime. Patients are able to receive treatment and return immediately to their everyday lives.

Dysport™ and BOTOX® are purified proteins produced by the Clostridium botulinum bacterium. Both Dysport™ and BOTOX® work by blocking transmission of nerve impulses to the muscles in the area. Dysport™ and BOTOX® effects are confined to the injection site and do not travel throughout the body.

Boston plastic surgeon Sean Doherty has found that Dysport™ and BOTOX® are popular among both male and female patients who have the following conditions:

  • Frown lines between the eyebrows (glabellar area)
  • Forehead creases (frontalis area)
  • Crow’s feet near the eyes (orbicularis oculi muscles)
  • Bands or cords in the neck (platysmal bands)

The Dysport™ and BOTOX® Cosmetic Procedure

Neurotoxins are administered directly to the target areas through a series of small injections. Discomfort is minimal and brief. Dr. Doherty will apply a topical anesthetic and an ice roller prior to the procedure to make the injection of either Dysport™ or BOTOX® the most comfortable experience.

After consultation with you, Dr. Doherty will determine where to place the neurotoxin injections for the best results. This is based on what areas are concerning to you. Side effects are few. Immediately after the injection, there may be swelling in the area treated. This subsides soon after the injection with the aid of cool ice packs. Rarely, some patients may bruise slightly at the injection site. This effect is easily covered with makeup and resolves in a few days.

Dr. Doherty recommends that Boston Dysport™ and BOTOX® Cosmetic recipients refrain from heavy physical exercise or facial massage for at least 24 hours after treatment as well as any use of non-tylenol pain medicine (i.e. aspirin or ibuprofen)

Within a few days of a neurotoxin injection, patients see a marked improvement in their frown lines. Dysport™ tends to take effect in 24-36 hours and BOTOX® begins to show its effects in 3-5 days. Both neurotoxins may take up to two weeks to show their full effect. Both Dysport™ and BOTOX® can last up to four months. The Dysport™ Difference?

Dysport™, an alternative to BOTOX®

The FDA approved Dysport™ for cosmetic and medicinal use in the United States in June 2009. This purified type-A neurotoxin produced by the bacteria Clostridium botulinum and temporarily prevents the release of acetycholine from sympathetic nerve terminals, This results in a smoothing away of  furrows between the brows.

The onset of Dysport™’s muscle relaxing effect may be faster than BOTOX® and may last longer.

Just like BOTOX® Cosmetic, small doses of the toxin  are injected into targeted wrinkle-causing muscles preventing the muscle from contracting and effectively reducing the wrinkle’s appearance. Also like BOTOX®, Dysport™, doesn’t require much downtime for recovery. The patient can return immediately to his/her natural activites, play or work.

This new wrinkle relaxer gives patients another choice. Botox® Cosmetic injections ranks in the top five nonsurgical cosmetic procedures in the United States.  Choice is good for patients’ health and budgets.

Dysport™ has been available in the U.K. since the early 1990’s and is licensed in 67 countries worldwide including the United Kingdom and many European markets. Dysport™ continues to be studied, in particular for its reach or diffusion rate—considered to be greater than BOTOX®, able to spread to muscles not intended for treatment. Dr. Doherty is very comfortable using Dysport™ to treat facial wrinkles and looks forward to the possibility of discussing the benefits Dysport™ or BOTOX® may provide to you.

Injectable Skin Fillers

Skin is made up of two layers. The outer most layer of skin is called the epidermis. It is a protective layer that controls the loss of water from our bodies. Below the epidermis is the dermis which is made up of collagen—a protein. Collagen forms the network of support of our skin.

When skin is young, the collagen framework is strong, healthy and intact. This allows the skin to be moisturized and elastic. Young skin is strong and resilient. Lines that appear when smiling or frowning disappear after smiling. This is because the skin is elastic. Over time, the collagen framework weakens and skin tends to lose its elasticity. Older skin is less resilient, so the facial lines that appear with a smile or frown do not disappear when there is no smiling or frowning.

While surgery remains the best way to achieve long-lasting results, soft tissue skin fillers can create significant improvement in patients of most ages. Patients who come to our practice for facial fillers report improvement in the following areas:

  • Wrinkle reduction
  • Increase in tissue volume
  • Skin contour improvement in areas of acne scarring

Soft tissue augmentation using fillers injected just below the skin’s surface may add volume to the face and smooth deep lines and crevices that no not respond to surface treatments. Hyaluronic acids and calcium  hydroxylapatite produce an immediately visible difference in the skin’s appearance. Filler injections with eventually lose their form and wear down. Touch ups and repeat treatments are required to maintain the desired result over time.

RESTYLANE®, commonly used to fill such areas as the lips and nasolabial folds, is a natural dermal filler containing hyaluronic acid. This substance occurs naturally in the human body. It delivers nutrients and hydrates the skin. RESTYLANE® uses a biodegradable form of hyaluronic acid that is non-animal based. Unlike previously used collagen, no allergy tests are required before treatment.

RESTYLANE® is most often used to correct facial lines like nasolabial folds, marriorette lines and some smile lines. In addition, it is often used to increase volume in patient who are concerned with thin lips. For most patients, RESTYLANE® is effective for four to six months before repeat treatment is required.

Depending on the size of the area to be treated, RESTYLANE® can be applied in combination with other cosmetic procedures. Side effects are usually minor and may include temporary swelling and redness at the injection site.


Perlane® is a “mid-dermal” hyaluronic acid filler product. As a sister product to RESTYLANE®, Perlane® is designed for more dramtic  facial folds and wrinkles. Perlane® is injected into the deep dermis to superficial layer of the subcutis to add volume to restore surface contour in facial wrinkles and folds. Perlane® corrections maintain volume augmentation for about six months before natural product degradation begins to lessen the effects. Both Perlane® and RESTYLANE® are composed of hyaluronic acid. The primary difference between RESTYLANE® and Perlane® is that the gel particles in Perlane® are larger than those found in RESTYLANE®. This gives Perlane® additional ability to lift and fill the areas of injection that require more volume. Traditionally these areas are the nasolabial folds or the mid-face.  Perlane® and RESTYLANE® are perfect complements to one another. Also distinct from other hyaluronic acid dermal fillers, RESTYLANE® and Perlane® are minimally cross-linked, making them similar to the natural hyaluronic acid found in the body.


JUVEDERM® dermal filler is a hyaluronic acid dermal filler that  contains a  high concentration of cross-linked hyaluronic acid. This attribute provides optimal results with a single treatment of JUVEDERM® in the majority of patients and deliver sustained results for up to six months or longer. JUVEDERM® dermal filler is the only hyaluronic acid dermal filler developed using a proprietary, advanced manufacturing process that results in a malleable smooth gel that flows easily into the skin, creating a smooth, natural look and feel. All other currently approved hyaluronic acid dermal fillers utilize a gel particle suspension formulation. Other currently approved hyaluronic acid dermal fillers utilize a gel particle suspension formulation. JUVEDERM® is an excellent treatment option for:

  • Lip enhancement
  • Deep folds
  • Facial lines
  • Wrinkle correction
  • Volume augmentation of the lips
  • Shaping facial contours (such as cheeks and chin correcting)
  • Smile lines (correcting thin, superficial lines around the eyes, mouth and forehead)

An advantage of JUVEDERM® wrinkle filler is the two levels of product formulation which provide solutions for deep folds and lines, moderate wrinkling and the fine lines found around the lips and for some patients, the eyes.

  • The two  product formulations include:
  • JUVEDERM® Ultra dermal filler Ultra is a highly cross-linked formulation for more versatility in contouring and volumizing of facial wrinkles and folds;
  • JUVEDERM®Ultra Plus dermal filler Ultra Plus, a more highly cross-linked robust formulation for volumizing and correction of deeper folds and wrinkles;


Radiesse® differs from hyaluronic acids  in its basic composition. Radiesse® behaves like a semi-solid soft tissue implant. The principle component of Radiesse® is a synthetic calcium hydroxylapatite. This is a biomaterial with more than twenty years of use in other surgical specialties. Once injected into the skin, the calcium hydroxylapatite remains stable long after its gel carrier is reabsorbed. This results in a long term restoration and augmentation of volume to soft tissue.

Radiesse® has longer-lasting results than hyaluronic acid-based fillers, with results sometimes lasting over a year. Radiesse® is most suitable for deeper soft tissue volumization in the nasolabial folds, cheeks, and the frown lines. It is often used in combination with a “softer” filler such as RESTYLANE®, Perlane® or JUVEDERM® to combine long-lasting deeper tissue augmentation with a smooth surface.

Every patient who desires facial fillers to treat lines and volume loss is different. Dr. Doherty enjoys speaking to patients about their interests and goals in facial rejuvenation. He then works closely with them to determine the best treatment regime.

Laser Skin Resurfacing

Skin that has become rough or blotchy can be easily refreshed through laser skin resurfacing. This cosmetic procedure uses a laser to remove damaged layers of skin. As new skin cells develop, they present a firmer, more youthful appearance. Fine facial lines, age spots, sun spots, and other uneven areas of pigmentation can also be successfully treated with this procedure.

Dr. Doherty has extensive experience in the use of non-ablative and ablative skin resurfacing. Utilizing the Palomar® Starlux® platform, Dr. Doherty performs ablative, non-ablative or a combination resurfacing technique. The non-ablative laser used in the office is a 1540nm laser. Dr. Doherty and his staff use this laser for plumping and rejuvenation of the dermal  layer of the skin. The dermis is the strength layer. When this is treated with a non-ablative treatment, it’s strength is improved and the skin on the surface appears firmer and plumper. Dr. Doherty also uses this laser to treat scars. Appropriate scar care with the 1540nm usually requires at least four  treatments done every 3-4 weeks.

The Palomar® Lux2940™ nm ablative laser is what Dr. Doherty uses for patients who wish to have more significant facial skin resurfacing. Healthy patients who are concerned with fine lines, pigment irregularity and overall skin texture issues are excellent candidates for a Lux2940™ nm facial resurfacing procedure.  Dr. Doherty performs this treatment in the office with topical skin anesthetic. Patients are required to do skin soaks on the treated area for 3-4 days after the treatment. At this point, patients have a light pink color to their treated skin that may last up to 7-10 days. Patients may easily cover this up for mineral concealer makeup.

The “Combo” procedure that Dr. Doherty is performing in his office is a combination resurfacing treatment For appropriate patients, Dr. Doherty performs both non-ablative and ablative therapy in the same day. Dr. Doherty has found that this combination treatment provides for even more rejuvenation of the skin. There is significant overall skin plumping, improvement in fine lines, improvement in facial pigment and even some skin tightening. Both Dr. Doherty and his patients are thrilled with the results.

This “Combo” procedure is often tailored for each patient. Some patients require several 1540nm non-ablative fractional treatments done PRIOR to the ablative/non-ablative combination procedure. These are done four weeks apart in the months prior to the combination procedure. These extra treatments provide more collagen building and fibroblast production that helps improve overall skin results.


Dr. Doherty is most impressed with the results of his patients that have facial neurotoxin  treatment with either BOTOX® or Dysport™ and then two weeks later undergo a Combination Ablative/Non-ablative Laser Resurfacing. Dr. Doherty has found that when facial wrinkles are relaxed with either Dysport™ or BOTOX®, the laser resurfacing is even more effective. The results are amazing and his patients are so pleased with their results.
