Midface fillers are one of the most effective ways to address facial volume loss and restore youthful contours. As we age, changes in fat distribution and skin support can lead to hollowing in the cheeks and midface, making the face appear tired or less defined. Fat is often portrayed as the enemy by the media, but it can also be your friend. Patients who were teased for having a “baby face” when they were younger often get the last laugh as they age, since loss of fat in the mid-face and cheeks is one of the primary drivers of facial aging. A full face is a youthful-looking face.
The midface plays a key role in overall facial balance, providing proportion between the forehead and the jaw. Since facial rejuvenation is largely about restoring this balance, even a slight decrease in cheek volume can noticeably impact how you look and feel. Dermal fillers can be an excellent option for restoring this volume—but with so many choices available, how do you know which is the right filler for midface volume loss?

What Is a Midface Filler?
A midface filler is a dermal filler designed to restore volume in the cheek and midface region, helping to improve facial balance and reduce signs of aging caused by volume loss. The midface includes the area beneath the eyes and around the cheekbones, which plays a key role in maintaining a youthful contour.
Midface fillers work by adding structure, lifting the cheeks, and softening transitions between the lower eyelid and upper cheek.
What Causes Midface Volume Loss?
Midface volume loss occurs naturally with aging and is one of the primary reasons the face begins to look tired or sunken. Several factors contribute to this process:
- Fat pad loss and descent: The natural fat pads in the cheeks shrink and shift downward over time
- Bone resorption: The facial bones gradually lose density, reducing structural support
- Collagen decline: Skin loses elasticity and firmness
- Skin laxity: The skin becomes looser, exaggerating volume loss
This makes midface filler treatments a key solution for restoring volume loss and improving overall facial balance.
Why a Personalized Consultation Matters for Midface Fillers
A personalized consultation is one of the most important steps in achieving a natural, balanced result with midface fillers. It allows for a detailed evaluation of your:
- Skin texture
- Degree and severity of skin laxity
- Baseline anatomy
This information helps guide a treatment plan tailored to your individual:
- Facial structure
- Goals
- Lifestyle, including cost and tolerance for downtime
Taking all of these factors into account ensures the right filler—and the right approach—is selected for each patient.
Best Fillers for Midface Volume Loss
When choosing the best midface filler for volume loss, it’s important to understand that different products are designed for different purposes—some provide structural lift, while others allow for more natural movement.
Here is a quick comparison of the most commonly used options:
| Filler | Type | Duration | Best For |
|---|---|---|---|
| Juvéderm Voluma ® | Hyaluronic Acid (HA) | Up to 18–24 months | Deep structural support and lift |
| Restylane® Lyft | Hyaluronic Acid (HA) | 12–18 months | Contour and definition |
| Restylane® Contour | Hyaluronic Acid (HA) | ~12 months | Natural movement and flexibility |
| Sculptra® Aesthetic | Biostimulant | 2+ years (gradual) | Collagen stimulation and long-term volume |
Each of these fillers can be effective for treating midface volume loss, but the ideal choice depends on your anatomy, goals, and how much lift or flexibility is needed.
Midface Filler Options Explained
In my practice, I use several different fillers for the midface, each with unique properties that allow me to tailor treatment to the individual patient. Rather than relying on a single product, I often combine fillers to achieve a balanced, natural-looking result.
Benefits of Restylane® Contour
XpresHAn combines hyaluronic acid (HA), which is a naturally occurring sugar in the body, and crosslinks it in a specific manner and pattern to create a smooth, injectable gel that increases volume without stifling your tissue’s ability to form expression. Like Restylane® KYSSE for the lips, the HA in this filler actually binds to itself. This is what allows it to stretch out with your facial expressions and then retract back into its former space. Bottom line, I can restore volume while still allowing for dynamic movement. The result is a softer, more natural look that lasts for up to a year or more. For mid-face volume correction, I find it best to combine Restylane® Contour with one of the more robust fillers such as Restylane® Lyft or Juvéderm Voluma®.
Juvéderm Voluma®
Juvéderm Voluma® was the first HA filler on the market specifically designed to address mid-face/cheek volume loss. Made with a heavier-weight HA molecule than any of the other fillers in the Juvéderm line, it can be an ideal option for this area of the face in the right patient.
Sculptra® Aesthetic Not an HA Filler
The last option for restoring volume loss in the cheeks and mid-face is Sculptra® Aesthetic. It works in an entirely different way than Restylane® Contour, Restylane® Lyft, and Juvéderm Voluma, which are all HA-based fillers. Sculptra® Aesthetic, on the other hand, is a bio-stimulant that increases volume by promoting your own body to produce more collagen.
In general, it will take a series of treatments to achieve your desired result. And though this result will not last forever, it is more “permanent” than what can be achieved with an HA filler. This can be good news or bad news, as one of the advantages of HA fillers is that if you do not like your results, the filler can be dissolved. This is not the case with Sculptra® Aesthetic, which is another reason to be really careful when choosing your injector.
As with any aesthetic procedure, an exemplary result is all about matching the right product or procedure with the right patient. Injectables, which include both neurotoxins like Botox® as well as dermal fillers, are one of my areas of expertise. I have over 15 years of experience with all of the different fillers on the market, so I understand their subtle differences. This is why you want to see a Board-Certified plastic surgeon or a Board-Certified dermatologist with a lot of experience with dermal fillers. It is a procedure that is often presented as simple on social media, yet delivering a safe, exemplary result involves technical skill, deep knowledge, and a keen artistic eye. This is a procedure where even a little bit of filler, placed incorrectly, can look disturbing.
Midface Filler Before and After: What to Expect
Midface filler before and after results are typically subtle but noticeable, restoring natural volume and facial balance. Most patients see immediate improvement in cheek contour and a smoother transition between the under-eye area and midface.
Mild swelling or bruising can occur initially, but this usually resolves within a few days. As the filler settles, results appear more refined, creating a rested and more youthful look.
Before treatment, patients often notice:
- Hollow or flat cheeks
- Volume loss beneath the cheekbone
- A pronounced under-eye to cheek transition
After treatment, common improvements include:
- Fuller, more lifted cheeks
- Softer facial contours
- Improved overall balance
Natural-looking results depend on both the type of filler—such as Juvéderm Voluma or Sculptra Aesthetic—and proper technique.
Ideal Candidate for Mid-Face Fillers
The ideal candidate for a filler procedure to the mid-face is any man or woman in good overall health who is bothered by:
- Sagging skin around the lower face and mouth
- Hollow area beneath the cheekbone
- Visible transition between the under-eye area and the upper cheek
My mid-face filler patients often come in complaining that they just look tired and “grey” all the time, even after a full night’s sleep. So, if you are feeling like you need a pick-me-up, come in to see me to discuss whether a mid-face filler might be right for you.
