Fillers like Restylane® are often the first step that many of us take in our quest to combat facial aging. With each passing year, the face experiences a breakdown in collagen, which is like losing the foundation under a house, as well as a decline in facial fat. This 1-2 punch can act as a perfect storm, leading to the appearance of fine lines, wrinkles, hollows and folds. The latter may be particularly prominent in the lower part of the face as it tends to move a lot as we talk, frown, laugh and eat. Fillers are a really simple way to rejuvenate the face by replacing this lost volume.
Restylane® is a hyaluronic acid or HA-based line of fillers. HA is a substance that occurs naturally in the human body. It delivers nutrients while also hydrating the skin thus restoring plumpness and youth. Restylane® uses a biodegradable form of hyaluronic acid that is non-animal based and does not require an allergy test before treatment.

Each is uniquely suited for a specific area. What makes them different from one another is the weight of their hyaluronic acid. Some consist of bigger molecules which make them ideal for larger areas of volume loss like those in the mid-face/cheeks while others consist of smaller molecules making them better for fine lines around the eyes, for example.
It’s important that your plastic surgeon have a deep knowledge of how each Restylane® filler works alone as well as in combination with other fillers and/or Botox™ and other neurotoxins. Filler that is placed incorrectly is not only unsightly, but can actually be dangerous. Restylane® is best for patients with mild to moderate wrinkles and folds. Dr. Doherty likes to use it to correct:
- the lines from the nose to the corner of the mouth (nasolabial folds)
- marionette lines around the mouth
- any hollowness under the eyes
- certain smile lines
In addition, he will often use Restylane® to naturally increase volume in patients who would like a bit more fullness in their lips.
Restylane Lyft® is a slightly “heavier” filler. Dr. Doherty likes to use this mid-dermal HA filler to treat deeper facial folds and wrinkles such as those in the mid-face, cheeks, temples and nasolabial folds. As the face loses volume with age, the cheeks begin to fall. Plumping them back up with filler can take years off your face. Furthermore, adding a bit of filler to the temples is a simple way to rejuvenate the entire eye area, making your eyes look more youthful, fresh and vibrant. Restylane Lyft® has also recently been FDA approved for hand rejuvenation, adding to its previous indication and approval for facial injections. This makes it ideal for treating wrinkles, and disguising prominent veins on the back of the hands, often a dead giveaway of a patient’s actual age.
Made with proprietary XpresHAn Technology ™, Restylane Defyne® crosslinks different weights of hyaluronic acid. This makes it best for smoothing out moderate to severe deep wrinkles and folds around the nose, mouth and chin without impairing a patient’s ability to make facial expressions. The beauty of this filler is that it provides structure without swelling. Dr. Doherty likes Restylane Defyne® because it allows him to address the more severe signs of aging without producing stiff, mask-like results. His goal is for the patient to look rested and more vibrant, not “done”.
Restylane Refyne® is also made with cross-linking XpresHAn Technology™. But this filler is slightly lighter in weight than Restylane Defyne®. This makes it better for mild to moderate deep wrinkles and folds, especially the nasolabial folds and marionette lines. Using too much or the wrong weight filler in these areas can make a patient look overstuffed. For some, the area above the lip can take on an almost simian quality. Maintaining a natural-looking result is all about using the right product, in the right amount, and in the right location.
As we age, our lips, like everywhere else on the face, begin to lose volume. In the past, there were not products specifically designed for the lips and the results were often less than ideal. Restylane Silk® was created to subtly increase volume in the lips and treat fine lines around the mouth. It uses smaller, smoother hyaluronic acid particles to deliver completely natural-looking results. Restylane® Procedure & Results
The Restylane® procedure, like any injectable treatment, is straightforward and fast. The whole process typically takes less than 30 minutes even if we are treating multiple areas. Dr. Doherty or his staff will first numb the treatment area or areas with a topical numbing cream and ice to reduce any discomfort. Also, the Restylane® injection itself contains lidocaine, a numbing agent, to make the whole experience as painless as possible.
Patients can expect some mild swelling and possible bruising in the treatment area for a few days following your procedure. There may also be some redness at the injection sites themselves, but this typically clears up in 24-48 hours and is easily masked with makeup. It takes about two weeks to see your final results so be patient. Depending on the type of filler used, these results can last anywhere between 4 months to a year. In general, the heavier the HA molecule, the longer the results will last.

Does Restylane™ deliver the same results as Botox®?
No. Confusion arises because both are typically lumped under the procedure heading “injectables”. This is because they are administered in the same fashion through a small needle injection. Also, most plastic surgeons, including Dr. Doherty, will use these two products together. But each does something unique. Botox® works by relaxing the underlying muscle that creates active wrinkles. These typically occur between the eyes (the dreaded 11s), around the mouth and on the outer corners of the eyes (crow’s feet). Restylane®, on the other hand, is a filler. It works by adding volume to a line, hollow or fold to restore plumpness.
Is Restylane® a better product than Juvéderm?
It’s not about one being better than the other. Allergan, the manufacturer of the Juvéderm line, and Galderma, the manufacturer of the Restylane® line, both make excellent fillers. The key to any successful filler procedure is matching the right filler to the right problem. Dr. Doherty will usually use a combination of fillers to treat a patient’s facial aging issues as each works in a slightly different way.
My friend just got Restylane Defyne® and looks great. Would this be right for me?
It’s impossible to say without examining you in person. This is why an in-person consultation is so important. Every patient is unique, and not just in terms of his or her anatomy. We all have different goals and different lifestyles. This not only affects how fast we age, but also the best treatment options for restoring a more youthful look. Restylane Defyne® is a heavier filler that is best for plumping up the mid-face and cheeks. But you might have a naturally fuller face than your friend. You might not need to add any fullness to your cheeks. Instead, your issue could be the “commas” at the side of the mouth. It is vital that any injectables treatment be individualized to the patient. It’s never one size fits all.
Does Restylane® hurt?
Not really. We do everything in our power to limit a patient’s discomfort. This includes topical numbing cream and ice. Furthermore, most injections include lidocaine which provides additional numbing. Having said that, certain areas of the face are more sensitive than others particularly the area around the nose. But most patients describe their Restylane™ treatment as being more uncomfortable than painful.
How soon before I look good?
At first, you are going to look slightly swollen. The actual injection sites may be red, and in very rare instances, there may be some post-injection bruising. The redness will clear up in about 24-48 hours and is easily covered with makeup. The swelling may take a bit longer, but, in general, most people won’t notice it. However, it takes about two weeks before the final results from your Restylane® treatment reveal themselves. So, patience is a must.
